First, contact the Social Worker/Case Manager at the hospital where your loved one is being treated. The Social Worker/Case Manager will work with the Admissions Coordinator to ensure that a bed is available and make sure the proper paperwork is faxed to our facility. Next, you may contact the Admissions Coordinator and schedule a meeting and/or tour and discuss any questions you may have upon admission.

What therapy programs are available at The Place at Pepper Hill?
The Place at Pepper Hill offers Physical, Occupational and Speech therapy for patients who are discharged from the hospital after meeting Medicare eligibility. Our therapists evaluate the needs of each resident when they are admitted to The Place at Pepper Hill and determine which therapeutic area(s) they need to concentrate on.
